This is the 'Location' template. The template shows a map with a marker at the specified position.

{{SmallMap | Coordinates=longitude, latitude}}

You can use the Form:SmallMap to add it to your user page or just copy it to your user page. With this form you can select the position of the marker on a map instead of giving the coordinates in longitude and latitude.
