This is the "SmallMap" form. It can be used to add the Template:SmallMap to a page.

Enter page name below. If the page does not exist it will be created.

The template shows a map with a marker at the specified position. With this form you can select the position of the marker on a map instead of giving the coordinates in longitude and latitude.
